What is topiramate?

Topiramate is FDA-approved for seizure disorders and migraine prevention. When prescribed alone for weight management, its use is off label. Qsymia combines phentermine with a specific extended-release topiramate formulation and is FDA-approved for chronic weight management in eligible patients.

Why it may affect weight

Topiramate may reduce appetite, cravings, reward-driven eating or the appeal of certain foods. Some patients find it useful when cravings or evening eating are prominent, but response varies and the cognitive burden can outweigh benefit.

Important safety considerations

  • Tingling, altered taste, fatigue or dizziness
  • Word-finding difficulty, slowed thinking or impaired concentration
  • Mood changes or suicidal thoughts
  • Metabolic acidosis and the need for bicarbonate monitoring when indicated
  • Kidney stones, reduced sweating and heat illness
  • Acute eye pain or vision change requiring urgent evaluation
  • Fetal risk and careful pregnancy planning
Do not stop topiramate abruptly. Sudden discontinuation can precipitate seizures even in someone without a prior seizure diagnosis. A clinician should direct tapering.
