Medical coverage and pharmacy coverage are different

An insurance plan may cover the W8MD medical visit while excluding obesity medication from the pharmacy benefit. Conversely, a drug may appear on a formulary but require prior authorization, step therapy, quantity limits or continued-response documentation.

Why requests are denied

Common reasons include a plan exclusion, unmet BMI or comorbidity criteria, missing documentation, required step therapy, nonformulary drug, incomplete renewal data or submission under the wrong benefit. A denial is not always appealable, especially when the employer excluded the drug class.
